On behalf of (End User's Company), I am taking delivery of the above listed LIFT-ALL aerial device. I have been presented with both a LIFT-ALL Maintenance and Repair Parts Manual and an Operations Manual, and understand the importance of proper maintenance and safe operating practices of this unit. I also understand that anyone operating this unit must be informed of proper safety practices to be followed as explained in the Maintenance and Operations manuals.

 

I have received and understand the LIFT-ALL Limited Warranty. Furthermore, I understand this warranty is not valid until this Warranty Registration is received signed by Hydra-Tech, Inc., manufacturer of LIFT-ALL aerial devices. I understand it is the responsibility of the End User to either send the white copy of this Warranty Registration form to Hydra-Tech, Inc., P.O.Box 9738, Fort Wayne, IN 46899 or this Online form submitted upon agreement and fulfillment on this web page. This must be completed within five days of receipt of the LIFT-ALL aerial device at the End User's facilities.
